§ 30-323. Application.

  • Any person desiring to establish, maintain or operate a bakery, subject to the provisions of this article, shall make application for such permit, in writing, at the office of the department of health on a blank furnished by such office. Such application shall set forth the name and residence of the applicant if an individual and the names and residences of all of members if any firm or the president, vice-president and secretary if a corporation, together with the location of the place for which such permit is desired. Such application shall also state the maximum number of persons to be employed in such bakery, the number of rooms or apartments therein and any other facts concerning the proposed bakery which the department of health may require to have stated in such application.
