§ 30-1639. Application process.
(a)
Each applicant requesting a permit pursuant to this division shall complete a standard application form prepared by the department of health including, but not limited to, the following information:
(1)
The name of the person who is applying for the permit; and if a corporation, the names of all officers of the corporation; and if a partnership, the names of all partners;
(2)
The legal and business address and telephone number of the applicant and contact person associated with the applicant;
(3)
The location of the business;
(4)
A description of each vehicle which will be used by such applicant for the transportation of solid waste and/or any other substance delineated in section 30-1636, including the make, model, year, license number, and name of registered owner if different from that of the applicant;
(5)
Such other information deemed necessary by the department of health to fulfill its processing, inspection and enforcement obligations under this article.
(b)
The applicant must certify that all information provided in the application is true and correct.
