§ 30-1083. Application for license.

(a)

Every person desiring to operate a nursing, rest or convalescent home shall file an application for license with the department of health.

(b)

The application shall be on a form prescribed and furnished by the department of health and together with such other information as the department shall require, shall state:

(1)

The name and address of the applicant and the name of the person in charge of such home and if the applicant is a copartnership, association or corporation, the application shall state the names and addresses of all the partners or officers, as the case may be;

(2)

The location of the proposed private nursing, rest or convalescent home;

(3)

The facilities of the private nursing, rest or convalescent home for the care, treatment and nursing of patients or inmates, including sanitary and fire protection facilities.
