§ 30-983. Contents of application.

Any person desiring a massage therapist permit shall file a written application with the department of health. The application shall be verified by the applicant. Any applicant for a massage therapist permit shall furnish all the information required by section 30-957. In addition, the applicant shall furnish the following:

(1)

Certificate from a medical doctor, licensed to practice in the state, stating that the applicant has, within 30 days immediately preceding the date of application, been examined and found to be free of any contagious or communicable diseases.

(2)

A diploma or certificate of graduation from a school of massage.

(3)

The massage establishment, if any, at which the applicant expects to be employed.
