Division 3. MASSAGE THERAPIST PERMIT |
§ 30-981. Qualifications.
Any person is qualified for licensure as a massage therapist under this division who:
(1)
Is at least 18 years of age and has received a high school diploma or graduate equivalency diploma;
(2)
Has submitted a sworn affidavit completed before a notary and two witnesses indicating that the applicant for a massage therapist license has completed a resident course of study of not less than 200 hours given in not less than three calendar months at a school or schools which have for their purpose the teaching of the theory, method and profession of massage therapy. Additionally, the applicant shall provide the name and address of the school or schools and such other information relating to the school or schools as may be required by the director of the department of health; and
(3)
Has received a passing grade on an examination administered by the department of health.
