§ 2-87.1. Submittal of board questionnaire prior to appointment.

Prior to an individual's appointment to any board, commission, authority or other public body as defined in this division (hereafter "board"), said individual shall submit a completed and notarized questionnaire on forms provided by the council research office. Failure to submit said questionnaire shall preclude appointment. Any material misrepresentation submitted on a questionnaire shall provide cause for removal of an appointed board member. The requirements of this section shall not apply to appointments of elected officials, nor shall they apply to security districts created and approved pursuant to LA. R.S. 33:9100.21.
