§ 130-56. Reports.

  • (a)

    It shall be the duty of every licensed secondhand motor vehicle body parts dealer to make out a report which itemizes each item of motor vehicle body parts either bought, bartered, sold or otherwise acquired from a seller.

    (b)

    Such reporting form shall be on carbonless duplicating paper with one original and two copies. The superintendent of police shall describe the manner and detail of such reports and make them available to each permittee. These reports shall include, but not be limited to, the following items of information:

    (1)

    Consecutively numbered forms.

    (2)

    Date of transaction.

    (3)

    Time of transaction.

    (4)

    Name of permittee's clerk or agent.

    (5)

    Name of seller.

    (6)

    Race, sex, height, weight, hair color and eye color of seller.

    (7)

    Address and phone number of seller.

    (8)

    Seller's birth date.

    (9)

    Seller's Social Security number.

    (10)

    Signature of seller.

    (11)

    Dollar amount of transaction.

    (12)

    A complete and accurate description of each item received by the permittee including:

    a.

    The type of vehicle, model year, manufacturer,

    b.

    Description of vehicle part, including color, identification number, and any other identifying features.

    A declaration signed by the permittee that the seller has produced valid photographic identification issued by either the state, another state or by the United States military which bears a likeness of the seller.

    (14)

    Seller's identification card number and issuing entity.

    (15)

    A declaration of ownership stating that the seller is in rightful possession of the items being offered to the permittee.

    (16)

    The permittee shall cause to be made a right thumbprint of the seller.

    (17)

    The permittee shall cause to be made on each motor vehicle part included on the reporting form, a permanently affixed indelible number, which shall be the individual voucher number on the reporting form provided by the department of police. The permittee shall make available to the department of police or any authorized city employee any motor vehicle parts and forms requested during normal business hours for inspection to determine that the procedures are being met.

    (c)

    The permittee shall then deliver the original copy of this report to the superintendent of police within 24 hours of the transaction. One copy shall be given to the seller and the remaining copy shall be retained by the licensee on the business premises for a period of at least three years and shall be made available to the superintendent of police who shall then cause them to be examined from time to time during normal business hours for the purpose of accounting for each report. This report shall be filled out in its entirety.
