Any person who wishes to obtain a mayoralty permit for the sale of cut flowers on
the public streets, sidewalks and other public places must file an application with
the department of finance on a form provided by the department. The application shall
state the true, lawful and complete name and address of the permittee, the trade name,
if applicable, the address or addresses of any locations used for the storage of carts,
equipment, supplies and/or inventory, and two city references. Such application must
be signed by applicant before a notary public. A separate application and permit shall
be required for each outlet.
